Floating Case Manager
We are in need of a Case Manager for formerly homeless men and women with a primary diagnosis of a chronic mental illness, who have a history of drug, alcohol, r substance abuse. These programs provide affordable housing units to formerly homeless men and women. The primary responsibility of the Case Manager is to provide direct case management support to formerly homeless men & women in safe havens, transitional, and permanent housing facilities. Specifically the responsibilities will include but are not limited to the design and implementation of goals with the residents; maintenance of case records & file systems; to locate appropriate community resources that will assist residents in meeting their goals; to provide assistance with daily living skills; and act as a liaison with outside service providers. Qualified candidate must have a BA in Social Work or a related field, prior case management experience, excellent verbal and written communication skills, willingness to listen & respond to others with compassion, knowledge and experience with mental illness, issues regarding homelessness, & addictions, and strong computer skills (Microsoft Office applications). A valid driver's license is required and the ability to meet our insurance standards. Schedule: Monday - Friday 40 hours per week, with possible weekend's on-call. Hours: The hours will vary as some evenings are required.
