Case Manager - Hope Haven

Hope Haven Diamond Street residences provide 20 permanent Single Room Occupancy (SRO) housing units for formerly homeless recovering men who have completed a drug or alcohol treatment program and who are seeking a supportive group living environment as a continuation of their recovery program. We are seeking an individual with skills to work with men in their efforts to maintain housing and develop their ability to live independently. The Case Manager prepares service plans that will focus on education, employment, health, and maintenance of recovery and/or management of mental illness. Additional job responsibilities include working with residents to ensure house rules are followed and apartments are maintained to standards along with the recruitment and screening of new residents

 

Qualified candidates must have:

•  A bachelor's degree in a related field;

•  Prior social work/case management experience, working with adults in recovery;

•  Excellent verbal & written communication skills;

•  Strong organization and computers skills (Microsoft Office Applications);

•  Valid driver's license, pass our van driving test, meet our auto insurance clearance;

•  Successfully obtain clearance of criminal background checks.

 

Schedule: M - F 9am - 5pm and on-call hours.
