Permanent Supportive Housing
Project H.O.M.E.'s Permanent Supportive Housing (PHPSH) offers an independent living situation by providing affordable Single Room Occupancy (SRO), efficiency, and 2, 3, and 4 bedroom units to individuals and families who require regular, but not around the clock, supportive services.
Residents living in Project H.O.M.E.'s permanent supportive housing come from
emergency shelters or transitional housing for homeless persons and have a
general pattern of stability in housing for approximately one year prior to
entering PHPSH. These residents pay 30% of their income for rent,
with the remainder covered by federal housing subsidies. Most of our
permanent supportive housing units are subsidized through the
U.S. Department of Housing and
Urban Development, through either the Section 8 program or the
Shelter Plus
Care program.

Hope Haven Residences
Hope Haven I is a permanent supportive housing residence for formerly homeless men who have completed a drug or alcohol treatment program and who are seeking a supportive group living environment as a continuation of their recovery program. Hope Haven I has 12 subsidized single room occupancy units. Residents are offered comprehensive services including on site case management, addictions counseling, health services, and educational and employment opportunities. Most residents are employed or engaged in education or vocational training. Residents pay 30% of their income for rent as well as purchase and prepare their own food. Various recreational activities are offered and there is a tenant council that is a vehicle for resident input into house rules and services.
Hope Haven II is a permanent supportive housing residence for formerly homeless men who have completed a drug or alcohol treatment program and who are seeking a supportive group living environment as a continuation of their recovery program. Connelly House
Connelly House - in partnership with Bethesda Project - provides critically-needed affordable housing to 79 formerly homeless men and women. Connelly House also offers case management support, community space, laundry facilities, a workout room and a computer lab complete with high-speed internet capability to assist with searches for educational and employment opportunities. Opened in 2010, The building is located in the center of Philadelphia's business district at 13th and Market Streets and was accredited by the American Institute of Architects award for Community Design and was certified by the U.S. Green Building Council LEED certification system for being an environmentally-friendly, sustainable building.
Project H.O.M.E.'s "In Community" Program at Connelly House is a clean and sober, supported independent living program consisting of 55 subsidized, semi-furnished, efficiency apartments for formerly homeless men and women with a primary diagnosis of a serious mental illness with or without a substance use disorder. Residents are offered comprehensive services including on-site case management, addictions counseling, health services, and educational and employment opportunities. Residents pay 30% of their income towards rent. Residents are also encouraged to participate in meaningful activities that lead to personal growth and overall well-being. Residents living at In Community also purchase and prepare their own meals. Various recreational activities are offered and there is a tenant council that is a vehicle for resident input into house rules and services. 1515 Fairmount Avenue is a permanent supportive housing residence for men and women with a primary diagnosis of a serious mental illness. The program provides 48 subsidized single room occupancy units to formerly homeless men and women. Residents are offered comprehensive services including on site case management, addictions counseling, health services, and educational and employment opportunities. Residents living at 1515 are committed to maintaining their recovery and pay 30% of their income towards rent. Residents are also encouraged to participate in meaningful activities that lead to personal growth and overall well being. Residents living at 1515 also purchase and prepare their own meals. Various recreational activities are offered and there is a tenant council that is a vehicle for resident input into house rules and services.
1523 Fairmount Avenue is a permanent supportive housing residence for men and women with a primary diagnosis of a serious mental illness. The program provides 6 subsidized apartment style units to formerly homeless men and women. Residents are offered comprehensive services including on site case management, addictions counseling, health services, and educational and employment opportunities. Residents living at 1523 are committed to maintaining their recovery and pay 30% of their income towards rent. Residents are also encouraged to participate in meaningful activities that lead to personal growth and overall well being. Residents living at 1523 also purchase and prepare their own meals. Various recreational activities are offered and there is a tenant council that is a vehicle for resident input into house rules and services.

Rowan Homes
Rowan Judson A provides one of the few permanent, supportive housing programs in the nation for homeless families with special needs. Rowan Homes Judson Street offers 13-two bedroom and 11-three bedroom apartments, as well as community space, a quiet courtyard and a playground. Rowan families are offered comprehensive services, including case management, adult and youth education, job placement and retention services and access to quality health care. Residents are also encouraged to participate in on site groups that focus on parenting, health issues for adults and children as well as recreational activities such as arts and crafts and cooking class for the children.
Rowan Judson B provides one of the few permanent, supportive housing programs in the nation for homeless families with special needs. Rowan Homes Judson street offers 5-two bedroom and 3-three bedroom apartments. Rowan families are offered comprehensive services, including case management, adult and youth education, job placement and retention services and access to quality health care. Residents are also encouraged to participate in on site groups that focus on parenting, health issues for adults and children as well as recreational activities such as arts and crafts and cooking class for the children.
Rowan Homes Diamond Street includes 8 four-bedroom apartments for larger families (up to six children), as well as community space and a quiet courtyard. Rowan families are offered comprehensive services, including case management, adult and youth education, job placement and retention services and access to quality health care. Residents are also encouraged to participate in groups that focus on parenting, health issues for adults and children as well as recreational activities such as arts and crafts and cooking class for the children.

Kate's Place
Kate's Place is a permanent housing residence for low- to moderate-income men and women. The 11-story building has 144 single-occupancy, fully-furnished efficiency apartments, including 35 units of supportive housing. Kate's Place is fully accessible to persons with disabilities, with nine apartments designated for mobility-impaired persons, and nine apartments designated for hearing-impaired persons.

James Widener Ray Homes
The James Widener Ray Homes is the first development of the Middleton Housing Partnership. This completely renovated property provides 53 units of permanent, supportive housing for formerly homeless men, women and children - including veterans - as well as for qualified low-income families from the Tioga area. The residence is a four-story building that features energy and resource conservation elements in both design and construction.
Ray Homes is in an area poised for revitalization, and the project was made possible through a transformational leadership gift of Leigh and John Middleton. The Middleton Housing Partnership supplied start-up capital funding and enabled Project H.O.M.E. to access federal stimulus dollars. The doors have recently opened thanks to a generous lead gift from the Raynier Foundation, and the residence is named after the Foundation's founder.
