The Stephen Klein Wellness Center Medical-Legal Partnership
The Stephen Klein Wellness Center Medical-Legal Partnership (SKWC MLP) is a collaboration between Project HOME and the Legal Clinic for the Disabled, Inc. (LCD) that provides on-site free high-quality legal services to low-income patients with physical disabilities. The goal of the project is to address the health-harming legal needs of SKWC’s patients. Services include:
- Social Security Benefits
- Medicaid/Medicare assistance
- Public Benefits
- Housing and Utilities Assistance
- Consumer Debt
- Domestic Violence
- Custody / Divorce / Support
- Long-term Planning Documents
- Employment issues
Attorneys from LCD work alongside healthcare providers and educate both patients and providers on common legal issues faced by low-income clients. In order to be eligible for services a patient must have a household income below 150 percent of the Federal poverty line and a physical disability. More information about LCD can be found here.
HOURS OF OPERATION
Daytime hours: Monday through Friday from 8:30 am to 5 pm
Evening hours: Tuesdays until 7 pm
SERVICES
The Stephen Klein Wellness Center will provide services to people of all ages, with and without health insurance, and will include:
- Primary medical care (including pediatrics and family planning)
- Integrated behavioral health care
- Dental care
- Pharmacy
- Phlebotomy (blood draws)
- Wellness classes/programs
- Mobile nurse street outreach services
- YMCA-operated fitness facility
- Physical therapy
- Legal clinic
- Emergency food pantry for patients
- Hospitality showers
- Referrals to specialty care
- Assistance with applying for health insurance
